Clinical Indications
The patient is a 48-year-old man who developed increasing problem with tremors in his hand, right greater than left, associated with difficulties for fine movements.
Exam shows tremor, right greater than left, associated with reduced rapid alternating movements and increased tone in the upper limbs, right greater than left.
Interpretation
Nerve conduction studies are normal.
The right ulnar mixed nerve silent periods were normal (SP1 onset at 15 ms and end a 30 ms; SP2 onset at 45 ms and ends at 150 ms; SP3 onset at 210 ms and ends at 270 ms; inconsistent LLR).
The right ulnar nerve cutaneous silent periods were also normal (SP2 onset at 60 ms and ends at 174 ms; SP3 onset at 125 ms and ends at 350 ms; LLR responses at 52, 214 and 290 ms).
The right median H response had borderline normal latency for the patient’s height. It showed somewhat reduced habituation following repetitive stimulation at various low rates.
Needle EMG did not show neurogenic changes.
We recorded dynamic EMG using multi-channel recording simultaneously from the patient’s right biceps, right triceps, right flexor carpi radialis and right extensor digitorum communis muscles. We asked the patient to perform various tasks with his proximal and distal right upper limb and left upper limb. We also performed passive elbow and wrist flexion and extension maneuvers. We observed tremor of the alternating agonist antagonist type at a frequency of 5 Hz, superimposed on a less frequent synchronous postural tremor at a frequency of 6-7 Hz.
Conclusion
There is no evidence for neuropathy or radiculopathy involving the right upper limb.
The recorded tremor is consistent with an extrapyramidal etiology. The silent periods are normal, but the H-reflex habituation curves raise the possibility of a minimal dystonic component as well.
Referral to the neurology clinic is recommended for evaluation of an extrapyramidal disorder
